Cancellation of Lessons

    It is my hope that your lessons are a commitment and that you will find joy in them for many years, but I do understand that circumstances change and withdrawing from lessons may be inevitable. It is my hope that you will contact me to discuss any issues you may have so that we can explore options, but in the event that you wish to discontinue lessons for any reason, one month’s written notice is required. This form will be sufficient for the month's notice of cancellation of lessons. Please choose the final month below.
